Little Falls Village invites you to attend our COVID-19 Vaccination Clinic to be held
Friday, June 2, 9 am – 1 pm
at Sumner Village Community Center

Get either the Moderna or Pfizer bivalent booster and see friendly faces while doing it! As of April 19, 2023, CDC recommendations allow adults who are 65 or older or immunocompromised to get a second bivalent booster shot. This is the 8th vaccination clinic our Village is offering in partnership with Sumner Village and Giant Pharmacy.

(If you have trouble registering online, you may attend on a walk-in basis at the times noted below.
ARRIVAL TIME:
Although we do not anticipate any wait times, our recommended time for your arrival is based on surname as follows:
Last Name between A and K: 9 am – 10:30 am
Last Name between L and R: 10:30 am – 12 Noon
Last Name between S and Z: Noon – 12:30 pm
GIANT PHARMACY CONSENT FORM (attached).
Please print, complete, and bring the Giant Pharmacy Consent Form to the clinic to save time at the clinic. If this is too difficult, we will have this form at the clinic and Village volunteers available to help.
WHAT TO BRING TO THE CLINIC
1. Completed Giant Consent Form (will be sent by email prior to the clinic and available at the clinic as well)
2. Medicare Card / Insurance Card (photocopy preferred)
3. State issued ID (photocopy preferred)
4. Prescription Card (if you have one; photocopy preferred)
5. COVID Vaccine Card documenting your past doses of Covid vaccines
–If you do not have your Covid vaccine card, you can get documentation online from the Maryland Department of Health at the following link:

(If you do not have documentation of your COVID vaccines/booster, the pharmacist can look it up on site, but you may need to wait longer for the pharmacist’s help.)
VACCINE TYPE:
The Giant Pharmacists will bring a sufficient amount of both the Moderna and Pfizer bivalent vaccine to meet all needs and/or preferences. After the booster shot, you will be asked to wait for 15 minutes in the Library. If you are feeling well after 15 minutes, you may leave. If you would like to linger and chat (and we hope you do!), please chat outside (as space inside is limited)!
PARKING & TRANSPORTATION:
Parking is limited at the Community Center and overflow parking will be on Sentinel Drive and in the Safeway Shopping Center lot.
ADDITONAL INFORMATION:
A Post-Survey. After your vaccination, one of our volunteers will try to wave you down for a quick anonymous survey. The National Council on Aging (NCOA) has requested Little Falls Village offer a survey to our vaccine clinic participants. It is not obligatory, but we do hope you will participate! More details about this new recognition of Little Falls Village by the NCOA will be coming soon.
Vaccination Photos. A Little Falls Village volunteer may also request to take a photo. Again, please say ‘no’ if you are at all uncomfortable. However, both the NCOA and Little Falls Village would like to get the word out that regular vaccinations are an important part of looking after the health and well-being of the community! Photos and quotes from participants will certainly help.
